Clarification Notice on Government Subsidies PMA Pre-Assessment Form

We would like to issue an important clarification to all members of the public (MOP) regarding the use of the Government Subsidies PMA Pre-Assessment Form issued by us.

This form is intended solely as an internal tool to help our customers better understand their potential eligibility for mobility-related assistance schemes. It is not an official document recognised by healthcare institutions, care providers, or government agencies for grant or subsidy applications.

We have received feedback that some individuals may have attempted to present this form directly to hospitals or other care providers when applying for government support. Please note that:

• The MOBOT-issued pre-assessment form is not to be used as part of any official submission.
• Formal grant or subsidy applications must be made through the relevant government channels or authorised care providers.
• Only the official forms and documentation required by those agencies will be accepted.

We encourage customers who require financial assistance for Personal Mobility Aids (PMAs) to consult the appropriate healthcare professionals or government websites for the correct application process.
